This guidance document is a supplement to the Medical Summary Report Interview Guide and Template. It provides important considerations when working with individuals who are Deaf and hard of hearing. It includes helpful information about hearing loss, Deaf culture, and important accommodations. Finally, it includes sample questions that may be helpful in your interview.
Here is some guidance from the SAMHSA SOAR TA Center and the Social Security Administration for gathering information about and documenting Long COVID for SSI/SSDI applications.
The Listings are SSA’s categorized lists of illnesses and conditions and the specific severity criteria – symptoms, duration, and impairments – of each illness/condition that must be met for a person to be considered disabled by the illness/condition. It is helpful for case managers to become familiar with these requirements.
The CE report should show not only the individual’s symptoms, laboratory findings (psychological test results), and diagnosis but also the effect of the mental disorder on the individual’s ability to function in personal, social, and occupational situations.
Completing the SSA-827 allows SOAR case managers to obtain the applicant’s medical records, in order to verify that they meet the medical eligibility criteria for SSI/SSDI.
An SSA ruling clarifies existing regulations which require that claimants and/or their authorized attorney or non-attorney representatives inform SSA about, or submit all evidence known to them that relates to their disability claim. In the process of collecting medical evidence, the DDS examiner may decide that there is insufficient information to make a determination about disability. In this case, the DDS examiner can request an evaluation from an outside source called a Consultative Exam (CE).
