Case Manager I Coastal Disability Advocates


Coastal Disability Advocates is seeking a full-time Case Manager I who will represent disabled homeless claimants in pursuit of Social Security Disability, SSI and Medicaid entitlements ensuring that all steps are aggressively taken to secure a timely, favorable outcome to claims. The Case Manager will also maintain a schedule of routine updates with the agencies census of awarded claimants to determine that they are receiving all appropriate social services and housing with object of reducing dependence on emergent care.

Homeless Case Manager (Disability Assistance) The McNabb Center


The McNabb Center is seeking a full-time Homeless Case Manager. PATH staff members provide outreach to homeless individuals in Knoxville and the Knox County area to identify those in need of outpatient mental health services. Staff members connect individuals to the appropriate mental health services and help those individuals work towards permanent housing options. The SOAR component of this position requires that all SOAR enrollees will receive the same services as PATH clients, as well as additional services directed at obtaining income for clients from SSI/SSDI utilizing the comprehensive SOAR process. This position encourages collaboration among human service agencies to improve access to SSI/SSDI by facilitating strategic planning at the state and local levels. Coordinates efforts to educate other agencies about the SOAR process. Acts as regional coordinator for surrounding counties in East Tennessee. Provides trainings, technical assistance, support, and encouragement to these agencies.

SOAR Specialist Integral Care


IntegralCare is seeking a full-time SOAR Specialist to complete SOAR applications and provide specialized training, advocacy and assistance to consumers who may be eligible and wish to apply for SSI/SSDI benefits. Services are provided to identified individuals who have experienced or at risk of homelessness accompanied by significant behavioral health, medical, and/or substance abuse issues utilizing a recovery-based and trauma-informed perspective.
