  • SOAR Benefits and Housing Case Worker

    The primary purpose of this role is to prevent and/or end homelessness through case management services and through assisting clients with accessing Social Security benefits through the SOAR model. Half of this position's purpose is dedicated to utilizing the SOAR model with eligible adults and children who are experiencing homelessness or at risk of homelessness and have a serious mental illness, medical impairment, and/or co-occurring substance use disorder. The other half of this position's primary purpose is to provide case management services to individuals and families who are experiencing homelessness or at risk of homelessness through the provision of time-limited, intensive support services and strategies to stabilize housing.
