Working with SOAR Online Course SSA Forms

Here is some helpful information if you are having trouble filling out and saving the SSA forms for the SOAR Online Course.

Saving and Completing SOAR Online Course Forms

Step 1:  Get Adobe

  1. You must have the most recent version of Adobe Acrobat Reader.

Step 2: Save the blank SSA forms to your computer

  1. Use only the forms that are provided on the “Meet Your Client” Practice Case page at the end of Class 1.  Look in the “Practice Case Materials” box.
  2. Right-click on each file, choose "Save Target As" or "Save Link As," and save in an easily-accessible location on your computer.
  3. If the file opened in the process of saving it, close it

Step 3: Open the form when you are ready to fill it out

  1. Open the file from the saved location on your computer.
  2. You may need to right-click and choose "Open with Adobe Acrobat Reader" to be sure the form is opening within the Adobe software.
    1. If it looks like you are on webpage or in Preview (on a Mac), this will not work. 

Step 4: Fill out and save the completed forms on your computer

  1. Type information into the forms and save your changes as you go.
  2. You may have to choose "Save As" and change the filename after entering information. (This is a good opportunity to add "Completed" or something similar to the filename so you can distinguish it from the blank forms you saved).

Step 5: Upload completed forms when prompted on a Practice Case page

  1. Click “Choose File," to find and select the completed form saved on your computer. 
  2. Once you have uploaded all required forms for the Practice Case page, click “Submit."
  3. If you upload an incorrect file, you can delete your submissions and re-upload the correct versions.

Step 6: Confirm the form uploaded correctly

  1. Under "Step 3: Upload Completed Files" click on the files you have uploaded to view them.
