This toolkit will help you to develop a SOAR referral process for your community or agency that helps ensure that potentially eligible individuals are connected with SOAR-trained providers who are available to assist.

The following stories were submitted with permission to the SOAR Technical Assistance Center in response to a call for examples of how Social Security disability benefits have changed the lives of people who were experiencing or at risk for homelessness.

This issue brief, developed by the SAMHSA SOAR TA Center, discusses best practices for integrating benefit options with community reentry strategies for people with serious mental illness and co-occurring disorders to promote post-release success.

Here are the SSA forms, links, and other helpful resources you will need to complete SSI and/or SSDI applications.

SSDI is an insurance program. Therefore, no information about living arrangement or resources is required. Instead, the SSDI application will ask questions about past earnings and entitlement to other benefits. People over 18 years of age who have worked and contributed to the Social Security Trust or others who became disabled before the age of 22 whose parent is deceased or receiving retirement or disability benefits may be eligible.

The SSA-3368: Adult Disability Report helps DDS to obtain a complete picture of the applicant’s medical history and treatment. Complete information is essential. Be sure to include all information available to you.
