Here are a number of resources for professionals as well as for individuals with brain injuries and their families. Incorporating SOAR screening and referral into your Coordinated Entry System helps prioritize access to benefits assistance for those who are most vulnerable. Helping those most in need to quickly obtain income increases your ability to place them in appropriate housing.

Children who have HIV/AIDS often have multiple disabling conditions, such as serious mental illnesses, cognitive disorders, and other chronic physical health conditions. Tracking SOAR outcomes is a critical way to document successes and to target technical assistance needs. Using the SOAR Online Application Tracking (OAT) program, case workers can easily enter application outcome data and generate reports to use for local sustainability efforts.
