As part of Kentucky’s TEAM SOAR (Team-Engaged Assertive Mechanics for SSI/SSDI Outreach Access Recovery) initiative — a SAMHSA-funded effort led by the Kentucky Department for Behavioral Health, Developmental and Intellectual Disabilities (DBHDID), Seven Counties Services, and the University of Kentucky Human Development Institute (UK-HDI) — Community Mental Health Center (CMHC) staff working with unsheltered individuals living with Serious Mental Illness (SMI), Serious Emotional Disturbance (SED), or Co-occurring Disorders (COD) are participating in a monthly Learning Collaborative.

The Learning Collaborative provides a structured space for CMHC staff to:

  • Strengthen best practices for serving individuals with SMI who are experiencing homelessness.
  • Share resources and strategies across different regions of Kentucky.
  • Build connections with colleagues working in similar professional roles, such as case managers, clinicians, and outreach specialists.

Participation is limited to CMHC staff engaged in outreach, engagement, and service provision for unsheltered individuals.
