Working with SOAR Online Course SSA Forms

Here is some helpful information if you are having trouble filling out and saving the SSA forms for the SOAR Online Course.

Step 1:  Get Adobe

  1. You must have the most recent version of Adobe Acrobat Reader.

Step 2: Save the blank SSA forms to your computer

  1. Forms are provided on the “Meet Your Client” Practice Case page at the end of Class 1.  Look in the “Practice Case Materials” box.
  2. Right-click on each file, choose "Save Target As" or "Save Link As," and save in an easily-accessible location on your computer.
  3. If the file opened in the process of saving it, close it

Step 3: Open the form when you are ready to fill it out

  1. Open the file from the saved location on your computer.
  2. Is the file opening as a webpage?  Rather than double-clicking to open it, right-click and choose "Open with Adobe Acrobat Reader."

Step 4: Fill out and save the completed forms on your computer

  1. Type information into the forms and save your changes as you go.
  2. You may have to choose "Save As" and change the filename after entering information. (This is a good opportunity to add "Completed" or something similar to the filename so you can distinguish it from the blank forms you saved).

Step 5: Upload completed forms when prompted on a Practice Case page

  1. Click “Browse” (or “Choose File,” depending on your internet browser) to find and select the completed form saved on your computer.
  2. Click “Upload."
  3. Once you have uploaded all required forms for that Practice Case page, click “Save and Continue.”
  4. If you upload the wrong file in error, click “Replace” and follow steps above to upload the correct file.

Step 6: Confirm the form uploaded correctly

  1. On any Practice Case Page, you can scroll down to "Your Practice Case Application Packet" and see the files you have uploaded for each class.
